Work Environment\n

This position allows for a flexible remote work setting, significantly reducing daily commute challenges and providing a work-life balance. You will be able to engage with clients directly from the comfort of your home while having access to necessary resources and support from our Hospice Center. Scheduled work hours are typically Monday to Friday from 8:00 AM to 4:30 PM, although some evening or weekend hours may be required based on client needs, underscoring the importance of flexibility in this role.

Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs, taking into account their emotional, mental, and social factors.

• Develop and review personalized care plans in collaboration with clients and their families, ensuring alignment with their goals and preferences.
\n
• Offer emotional support and counseling to seniors navigating life transitions, such as health changes, loss, and family dynamics.
\n
• Advocate for clients, ensuring they access necessary community resources and services aimed at improving their lives.
\n
• Coordinate care with interdisciplinary teams, including healthcare providers and caregivers, to provide seamless services.
\n
• Regularly visit clients to monitor their well-being, assess living situations, and ensure their safety needs are met.
\n
• Facilitate family meetings to improve communication and address any conflicts concerning client care.
\n
• Educate clients and their families on available resources that promote engagement and enhance their overall quality of life.

• Maintain accurate and timely documentation of client interactions, care plans, and ongoing assessments in our electronic health record (EHR) system.

• Stay current on social work ethics, policies, and best practices to ensure compliance and enhance service delivery.

As a Clinical Social Worker at our community center, you will primarily work with an aging population, addressing their unique psychosocial challenges. Qualifications\n

• A master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Certified Social Worker (LCSW-C).

• Exceptional assessment, counseling, and problem-solving skills, demonstrating empathy and understanding.

Compensation and Benefits\n

The salary range for this role is set between USD 60,000 and 100,000 annually, commensurate with experience and qualifications.
