Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of infants and toddlers to identify developmental delays and disabilities.

• Actively participate in the development and execution of Individualized Family Service Plans (IFSPs), ensuring a tailored approach to each child.
\n
• Monitor and document progress and outcomes, collaborating with team members to adjust strategies that facilitate optimal developmental growth.
\n
• Provide individualized support to families, offering guidance and resources that empower them in their caregiving roles.
\n
• Ensure compliance with all Federal and State regulations governing early intervention services, maintaining high standards of practice.
\n
• Build trusting relationships with children, families, and colleagues, fostering an encouraging and constructive learning environment.

• Design and implement treatment strategies using evidence-based practices and Natural Learning Environment Principles to effectively support child development.
\n
• Employ coaching models and natural environment teaching approaches to enhance family engagement and child learning.

Patient Population / Client Focus\n

This role focuses exclusively on infants and toddlers with disabilities or developmental delays, guiding their progress in a hospice center setting. As a Licensed Clinical Social Worker, you will work closely with families who may be navigating complex emotions and circumstances, providing the necessary support to foster resilience and growth during pivotal early years.

Qualifications\n

• A master’s degree or doctorate in social work.

• A valid Licensed Clinical Social Worker (LCSW) license as recognized by the Department of Public Health.

Compensation and Benefits\n
