Key Responsibilities\n

• Lead psychoeducational and process-oriented therapy groups to facilitate support and coping strategies for patients and their families.

• Conduct thorough clinical assessments to identify patients' mental health needs, tailoring interventions accordingly.

• Oversee case management duties, ensuring seamless coordination of care, including liaising with the Utilization Review and Business Departments regarding authorizations and billing processes.
\n
• Initiate discharge planning on the day of admission, setting goals for patient-centered outcomes with clear transition plans.
\n
• Complete discharge sessions to reinforce coping skills and resource availability, confirming discharge appointments with patients and their loved ones.
\n
• Collaborate with outpatient providers and referral sources during the entire patient journey to guarantee continued support and care continuity.

Qualifications\n

• Master’s degree in Social Work (MSW) from an accredited institution.

• Valid and current California licensure as a Clinical Social Worker (LCSW).

Compensation and Benefits\n
