Work Environment

This role takes place in a collaborative, onsite outpatient care setting within a school facility. As a Nurse Practitioner, you will work as part of a dedicated care team that includes physicians, physician assistants, medical assistants, care managers, and registered nurses. Our team approach allows for a holistic view of patient health and integrates care coordination with local specialists and hospitals to manage both routine and acute healthcare needs effectively.

Key Responsibilities

• Provide exceptional primary care to older adult patients, including comprehensive office visits for routine check-ups and acute health issues.
• Conduct annual wellness visits and health risk assessments, focusing on thorough documentation aligned with industry standards.
• Engage in care coordination with specialists, testing facilities, and external agencies to ensure seamless patient care.
• Lead initiatives related to population health, working with the care team on preventive screenings and health promotion strategies.
• Participate in phone triage and outreach efforts to connect with patients regarding their care plans.
• Educate patients on their health conditions and treatment options, empowering them to make informed choices about their care.
• Contribute to promotional activities aimed at raising awareness about our services and the importance of preventive health.
• Make home visits when necessary to provide care and check on patients’ well-being.
• Collaborate closely with the Medical Director and operational leadership on clinical initiatives and quality improvement.

Patient Population / Client Focus

Our primary patient population consists of older adults enrolled in Medicare. We specifically focus on delivering personalized and compassionate care that caters to their unique healthcare needs, aiming to enhance their quality of life and keep them actively engaged in their health strategies.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or graduation from an NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner recognized by AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license.
• Active DEA license to prescribe controlled substances.
• Previous experience in primary care settings, especially within internal medicine, geriatrics, or family medicine is essential.
• Knowledge of HCC documentation, ICD-10 coding, and health risk assessments is preferred.
• A strong enthusiasm for teamwork and a collaborative spirit is crucial for success in our integrated healthcare environment.

Preferred Experience

• At least two years of clinical experience in primary care, particularly working with a geriatric population.
• Familiarity with electronic health record (EHR) systems and telehealth platforms.
• Proven track record of effective patient communication and advocacy in clinical settings.
