Key Responsibilities\n

As a Licensed Mental Health Counselor, your responsibilities will include:

• Communicating regularly with physicians, reporting on patient progress, needs, concerns, and addressing any potential or actual issues that arise during treatment.

• Consulting and coordinating with family members and support systems to develop effective treatment and discharge plans tailored to individual client needs.
\n
• Maintaining accurate clinical records and progress notes in accordance with department policies, thus ensuring compliance and supporting ongoing care.

• Collaborating closely with members of the healthcare team to craft comprehensive treatment plans based on psychosocial assessments and validated social work interventions.

• Coordinating care during the discharge process, actively working alongside various service providers to facilitate access to necessary follow-up services.

Patient Population / Client Focus\n

Your expertise will serve a diverse patient population, including individuals experiencing a variety of mental health challenges such as anxiety, depression, and substance use disorders. Qualifications\n

The successful candidate will possess the following qualifications:

• A Master’s degree in Social Work, Counseling, Marriage and Family Therapy, or Psychology.

• Active state licensure as an LCSW, LMFT, LPC, or a comparable credential.

• A minimum of 3 years of experience in an outpatient mental health setting, demonstrating a commitment to effective patient care.

• Proficient in computer skills, particularly in word processing, email systems, spreadsheets, and electronic medical records (EMR) systems.

• The ability to work independently, be self-directed, and exhibit exceptional organizational skills to manage a caseload effectively.

Preferred Experience\n

Compensation and Benefits\n
