Key Responsibilities\n

• Conduct thorough virtual and in-person assessments to gather comprehensive information about clients’ mental and emotional health, ensuring the development of tailored treatment plans that address individual challenges.

• Utilize a variety of evidence-based therapeutic techniques and resources to effectively treat mental health issues, including anxiety, depression, grief, and trauma.

• Collaborate actively with an interdisciplinary team—including physicians, psychologists, and nursing staff—to coordinate care plans and ensure that clients receive a seamless continuum of care.

• Maintain diligent and accurate documentation of client assessments, treatment plans, and progress notes using our designated Electronic Health Record (EHR) systems.

• Act as an advocate for clients, facilitating access to necessary community resources and support systems, both virtually and in-person, to empower them and their families during difficult times.

Patient Population / Client Focus\n

The primary focus of this role is to provide support and counseling to patients within a hospice setting, along with their families. You will engage with individuals facing terminal illnesses and support them through emotional challenges, grief, and complex family dynamics. By developing trust and rapport with clients, you will play a crucial role in improving their quality of life during their final days.

Qualifications\n

• A master's degree in Social Work from an accredited institution is required.

• An active Licensed Clinical Social Worker (LCSW) credential

Compensation and Benefits\n

We offer a competitive salary ranging from USD 52,000 to 70,000 annually, commensurate with experience and qualifications.
