Key Responsibilities\n

• Conduct comprehensive assessments to determine the specific needs and treatment plans for each patient.

• Facilitate open communication among supervisors, direct care staff, families, and other stakeholders regarding individual progress.

• Develop and provide a structured schedule to supervisors, detailing appointments, home visits, and observations aimed at monitoring the treatment process.

• Coordinate and execute home visits that comply with in-home observation requirements, along with chart reviews and interviews to ensure thorough data analysis.

• Oversee the effective implementation of program plans, ensuring therapeutic interventions are appropriate and tailored for each client's needs.

• Engage in interdisciplinary team meetings to collaborate and strategize effective treatment approaches.
\n
• Document monthly and quarterly progress notes to accurately reflect client advancement.
\n
• Organize training sessions for staff to ensure they are well-versed in documentation practices and the implementation of program plans.

• Provide follow-up support via interviews and home visits to maintain continuity of care and address evolving client needs.

• Maintain meticulous records to ensure information is readily available to stakeholders, demonstrating transparency and responsibility.
