Key Responsibilities\n

As a Licensed Clinical Professional Counselor at Crystal Lake Families & Kids Services, you will:

• Assist primary care providers in recognizing, treating, and managing mental health and psychosocial issues as an integral member of the care team.

• Conduct comprehensive patient intakes, performing diagnostic and functional evaluations, followed by informed recommendations to the primary care provider regarding treatment goals and plans.

• Provide brief, focused therapy to patients requiring immediate mental health services, utilizing evidence-based practices to ensure effective interventions.

• Monitor patient progress through follow-up communications and adjust treatment approaches as necessary to enhance outcomes.

• Identify 'at-risk' patients and work proactively to develop and implement relapse prevention plans that promote stability in mental health.

• Document all patient interactions diligently within the treatment chart, maintaining accurate clinical records and ensuring proper communication with primary care providers.

• Educate patients on self-management techniques and healthy lifestyle choices to empower them in their own treatment and recovery processes.

• Collaborate with clinic management and other team members to evaluate behavioral health services and outcomes, contributing to ongoing program development.

• Maintain compliance with the Code of Ethical Conduct for Clinical Professionals, ensuring that all interactions remain confidential and professional.

Qualifications\n

The ideal candidate will possess:

Compensation and Benefits\n

Crystal Lake Families & Kids Services offers a competitive annual salary ranging from USD 87,000 to USD 105,000 based on experience and qualifications.
