Work Environment

This position is a full-time role that will require you to work onsite in the home care setting. You will travel to patients' residences, providing therapy in a comfortable and familiar environment. The standard work hours are Monday to Friday from 9 AM to 5 PM, offering you a healthy work-life balance.

Key Responsibilities

• Patient Assessment & Diagnosis: Conduct comprehensive evaluations to determine patients' physical conditions, including their range of motion, strength, and functional limitations. Utilize standardized assessment tools and clinical reasoning to inform treatment.
• Individualized Treatment Plans: Create and implement evidence-based treatment plans tailored to each patient's specific needs and recovery goals, adapting approaches based on progress and response to therapy.
• Hands-On Therapy & Modalities: Utilize a range of therapeutic techniques, including manual therapy, therapeutic exercises, and modalities such as ultrasound and electrical stimulation to restore function and alleviate pain.
• Patient & Family Education: Provide valuable education and resources to patients and their families regarding injury prevention, proper movement techniques, and the importance of adherence to home exercise programs.
• Progress Monitoring & Documentation: Maintain accurate and thorough patient records in compliance with state and federal regulations, tracking progress and modifying treatment plans as necessary to ensure optimal care.

Patient Population / Client Focus

As a Physical Therapist at InCore, you will work primarily with a diverse adult population, including seniors, post-operative patients, and individuals with chronic conditions. Many patients require rehabilitation following joint replacement surgeries, strokes, or injuries resulting from falls. Your role will be vital in guiding these patients through their recovery journeys and helping them regain independence in their daily activities.

Qualifications

• Licensed Physical Therapist (PT): Must hold an active New Jersey physical therapy license.
• Education: A Bachelor's degree in Physical Therapy or a related field is required; a Doctor of Physical Therapy (DPT) or Master's in Physical Therapy (MPT) is preferred.
• Experience: A minimum of 2 years of experience in physical therapy, particularly in an outpatient or home care setting, is essential for this role.
• Skills: Demonstrated proficiency in orthopedic rehabilitation with strong interpersonal skills, analytical thinking, and the ability to communicate effectively with patients and the healthcare team.

Preferred Experience

• Experience working with geriatric patients.
• Knowledge of and familiarity with electronic medical records (EMR) systems.
• Comfort in utilizing telehealth tools to enhance patient engagement and support.

Compensation and Benefits

InCore Physical Therapy offers a competitive salary ranging from $100,000 to $120,000 per year, commensurate with experience and expertise.
