Key Responsibilities

• Conduct comprehensive assessments to determine the counseling needs of clients, utilizing DSM-5 criteria for substance abuse diagnosis.
• Prepare individualized treatment plans that reflect the specific substance use counseling needs of clients and collaborate with the clinical team to coordinate care effectively.
• Monitor client progress through regular follow-up assessments and maintain thorough documentation in electronic health records, including educational services, referrals, and clinical interactions.
• Develop and implement discharge and continuum of care plans, ensuring a smooth transition for clients to the appropriate levels of care.
• Manage a caseload of clients, providing insightful case management updates and conducting utilization reviews as necessary.
• Coordinate with external support services, including legal and educational entities, to facilitate holistic care for clients and their families.
• Engage in crisis intervention strategies, utilizing a multi-disciplinary approach to address urgent client needs and challenges.

Patient Population / Client Focus

Compensation and Benefits
