Key Responsibilities

• Conduct thorough patient evaluations by reviewing medical history, performing specific assessments, and documenting findings to create individualized treatment plans that align with patient goals.
• Engage with patients and their families to explain evaluation outcomes and collaboratively set rehabilitation goals, ensuring their needs and preferences are prioritized.
• Implement treatment protocols effectively based on patient evaluations, ongoing assessments, and current industry best practices.
• Collaborate with multidisciplinary healthcare teams to discuss patient progress, recognize any changes in health status, and evaluate necessary adjustments to treatment plans.
• Oversee discharge planning processes and ensure appropriate follow-up care is established, maintaining accurate documentation of all patient interactions, evaluations, and care plans.
• Demonstrate strong time management skills to uphold productivity standards, maintain an organized patient schedule, and participate in hospital and community-based marketing initiatives.
• Contribute to departmental goals by adhering to regulatory and accreditation standards and actively participating in improvement initiatives.
• Pursue continuous professional development and education opportunities to enhance personal competency and enrich organizational capabilities.
• Deliver an exceptional patient experience through flexible and adaptable care approaches, actively working to remove barriers to access services.

Patient Population / Client Focus

Our clinic serves a diverse client base, ranging from athletes recovering from sports injuries to elderly patients seeking rehabilitation after surgeries or chronic conditions. Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.

Compensation and Benefits
