Key Responsibilities\n

  • Conduct thorough psychosocial assessments, evaluating clients' mental health concerns, strengths, and resources to tailor effective treatment strategies.

  • Utilize a variety of evidence-based therapy modalities, including cognitive-behavioral therapy, solution-focused therapy, and trauma-focused therapy to deliver individual, family, and group therapy.

  • Work collaboratively with other mental health professionals, such as psychiatrists and psychologists, to create and implement comprehensive, client-centered treatment plans.

  • Monitor treatment progress and make adjustments to care plans as needed to ensure optimal patient outcomes.

  • Provide crisis intervention and develop safety plans for clients in distress, effectively navigating high-pressure situations with professionalism and empathy.

  • Facilitate support groups and psychoeducation classes to empower clients and families with knowledge and coping strategies.

  • Maintain accurate clinical records and documentation in compliance with ethical and legal standards, ensuring confidentiality and integrity of client information.

Patient Population / Client Focus\n

As a Clinical Social Worker in our hospice center, you will primarily serve patients facing terminal illnesses and their families. This requires not only clinical expertise but also a deep understanding of the emotional and psychological complexities involved in end-of-life care. You will help patients navigate their feelings of uncertainty, fear, and grief, offering support that facilitates a dignified and peaceful transition.

  • A Master’s degree in Social Work from an accredited program.

  • Possession of an active state licensure as a Licensed Clinical Social Worker (LCSW).
