Work Environment

This position requires on-site engagement, where you'll visit patients' homes to evaluate and implement therapy sessions. Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions by reviewing medical histories, performing physical assessments, and setting collaborative treatment goals.
• Develop and implement evidence-based treatment plans utilizing a variety of therapy techniques tailored to each patient's specific needs, such as manual therapy, exercise prescriptions, and functional training.
• Communicate evaluation results and treatment strategies clearly to patients and their families, ensuring that they understand and engage with their rehabilitation process.
• Consistently monitor patients' progress, recognize changes in their conditions, and adjust treatment plans as necessary, coordinating with other healthcare professionals when needed.
• Document patient care thoroughly, including detailed initial evaluations, progress notes, and discharge summaries that comply with our clinic's standards and regulations.
• Participate actively in team meetings, contribute to continuous quality improvement initiatives, and take part in departmental goals aimed at enhancing the delivery of patient care.
• Engage in marketing activities to promote our services within the community while building relationships with local healthcare providers.
• Facilitate discharge planning and ensure that patients understand follow-up care and home exercise programs to maintain their progress.
• Dedicate time to professional development, pursuing additional education and certifications, and mentoring new staff or students as appropriate.

Patient Population / Client Focus

As a Home Health Physical Therapist, you will work with a diverse patient population, including postoperative patients, those recovering from injuries, and individuals with chronic conditions. Qualifications

• A Bachelor's Degree in Physical Therapy is required; candidates holding a Doctorate in Physical Therapy (DPT) will have an advantage.
• A valid Physical Therapist license in the State of Virginia is mandatory.
• Current CPR certification (BLS for Healthcare Providers) from the American Heart Association is required.

Compensation and Benefits
