Key Responsibilities\n

As a Licensed Clinical Social Worker, your primary duties will encompass:

• Comprehensively assessing the counseling needs of clients to develop tailored treatment plans.

• Utilizing DSM (Diagnostic and Statistical Manual) criteria to assess clients for substance abuse diagnoses, determining the appropriate level of care as guided by ASAM (American Society of Addiction Medicine) standards.

• Designing and preparing the substance abuse counseling components of client treatment plans based on individual assessment outcomes.
\n
• Creating client discharge and continuum of care plans to facilitate transitions into subsequent levels of care.
\n
• Accurately documenting all counseling sessions, educational services, assessments, and follow-ups within client clinical records to track progress and service efficacy.
\n
• Managing a caseload that requires optimizing case management activities, including regular utilization review updates and treatment planning documentation.

• Coordinating support services for clients and their families, ensuring comprehensive service delivery is recorded and effectively communicated.

• Maintaining regular contact with referral sources including legal, educational, and vocational entities to facilitate holistic client support.

• Implementing crisis-oriented interventions as necessary, working alongside a multi-disciplinary team to respond to emergent client needs.
