Key Responsibilities\n

• Deliver individualized mental health therapy to patients, including individuals, couples, families, and children, utilizing telehealth platforms to provide remote care.

• Assess patients' needs through comprehensive intake assessments, gathering pertinent history to inform treatment plans.

• Create, implement, and periodically review personalized treatment plans based on clinical best practices and client goals.
\n
• Use evidence-based therapeutic methods in both group and individual therapy settings to address various emotional, psychological, and social challenges.

• Collaborate closely with the Utilization Review Department and Business Department to navigate authorization processes and manage patient billing accurately.
\n
• Conduct community outreach initiatives to connect clients with local resources and support services.
\n
• Maintain accurate and timely documentation of therapy sessions, treatment progress, and evaluations in compliance with organizational policy and state regulations.

Patient Population / Client Focus\n

You will primarily provide support to patients and families in the hospice care setting, many of whom face terminal illnesses and require sensitive emotional guidance. This role requires not only clinical expertise but also a profound ability to empathize with issues surrounding grief, loss, and end-of-life discussions. Qualifications\n

• Master’s degree in Social Work from an accredited institution.

Compensation and Benefits\n

This position offers a competitive annual salary range of $120,000 to $130,000, commensurate with your experience and caseload.
