Work Environment

This position is located onsite within a school environment in Yonkers, where you will work closely with school staff, administrators, and parents to create a safe and health-conscious atmosphere. The Nurse Practitioner will have the unique opportunity to influence student health outcomes directly. Your work hours will be Monday to Friday, from 9:00 AM to 5:00 PM, allowing for a balanced work-life schedule while engaging with the school community.

Key Responsibilities

• Conduct thorough initial patient screenings, including detailed patient histories and physical examinations, to assess the health status of students.
• Maintain accurate and updated patient records during each visit, ensuring that charts comply with legal standards and best practices in medical documentation.
• Evaluate diagnostic test results, formulate treatment plans, and communicate the need for follow-up care to both patients and their families.
• Deliver high-quality, compassionate care while adhering to organizational productivity metrics and fostering a supportive clinic environment.
• Act as a vital resource for patients, providing ongoing information and counseling regarding their health, especially for those with chronic or complex conditions.
• Collaborate with school nurses and staff to implement health initiatives and participate in community outreach programs.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve students ranging from elementary to high school age within the Yonkers school district. This role requires a strong understanding of age-appropriate health issues, including developmental milestones, common childhood illnesses, and mental health challenges. You will play a crucial role in promoting wellness and preventive care strategies among this young population, aiming to mitigate health barriers to education.

Qualifications

• Current and valid Nurse Practitioner license in New York State.
• A Master of Science in Nursing (MSN) or a Doctor of Nursing Practice (DNP) from an accredited institution.
• Board certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) is required.
• A minimum of 2 years of clinical experience in an outpatient or primary care setting; experience with pediatric populations is highly preferred.
• Demonstrated ability in chronic disease management, preventive care, and effective patient education techniques.
• Strong diagnostic skills and a patient-focused approach to care delivery.
• Proficiency with electronic health records (EHR) systems for effective data management and communication.

Preferred Experience

• Experience working in school health settings or with children and adolescents.

• Conducting health workshops or educational sessions for students and staff.
