Key Responsibilities

• Conduct thorough psychosocial assessments to identify and address the emotional, social, and financial needs of patients.
• Provide individual and group counseling sessions focused on helping patients navigate adjustment challenges, grief processes, and coping strategies.
• Develop comprehensive discharge plans in collaboration with interdisciplinary teams, ensuring a smooth transition from hospital to home or community care.
• Offer emotional support to patients and their families, educating them about treatment options and available resources.
• Coordinate with community organizations to set up necessary post-discharge services to promote continuity of care.
• Participate in hospital committees to provide insights on social work practices and advocate for patients’ needs.
• Engage in crisis intervention as required, providing immediate support in high-stress situations to prevent escalation and ensure patient safety.
• Maintain accurate, timely documentation adhering to legal and organizational standards to ensure compliance and continuity of care.

Qualifications

Compensation and Benefits
