Work Environment

This role is full-time and entails an onsite work schedule focused on delivering home care physical therapy services. As a therapist on our team, you will travel to patients' homes in Punta Gorda and its surrounding areas. You will be responsible for assessing, developing, and implementing treatment plans that align with each individual patient's needs, all while collaborating with a multidisciplinary team to ensure a seamless patient experience. Your flexible and adaptable approach will enable you to meet our patients where they are, both physically and emotionally.

Key Responsibilities

• You will conduct thorough evaluations, assessing the patient's medical history and physical limitations, and using specific tests to develop tailored rehabilitation strategies.
• Effectively communicate the evaluation findings and treatment plans to patients and their families, actively involving them in goal-setting for better engagement.
• Demonstrate proficiency in applying treatment protocols based on each patient's specific needs and continually adjusting plans as their conditions evolve.
• Collaborate with other healthcare professionals to provide comprehensive care, recognizing when modifications to the plan are necessary.
• Maintain accurate, timely documentation of patient assessments, treatment plans, progress notes, and discharge summaries to enhance continuity of care.
• Balance patient loads and manage time efficiently while adhering to productivity standards, attending meetings punctually, and participating in community engagement activities.
• Contribute to departmental goals, ensuring compliance with healthcare regulations and accreditation standards.
• Take initiative for your professional development and that of others, possibly serving as a clinical instructor for students.
• Promote exceptional customer experiences by being adaptable and responsive to patient needs.
• Engage in initiatives to eliminate barriers to access, using relevant data to enhance service delivery.

Qualifications

• Must hold a Bachelor's Degree in Physical Therapy or related field.
• A valid state license to practice as a Physical Therapist in Florida is required.
• American Heart Association's BLS for Healthcare Providers CPR certification is mandatory.

Preferred Experience

While we require at least 2 years of clinical experience in physical therapy, we believe that dedicated professionals can excel and grow in this role with our supportive training and mentorship. Experience in home care or outpatient settings is advantageous but not mandatory.

Compensation and Benefits
