Work Environment

This position is based in our onsite rehabilitation center in Punta Gorda, a vibrant community known for its beautiful waterfront and outdoor recreational opportunities. Our facility is designed to optimize patient recovery, equipped with advanced treatment technologies and a welcoming atmosphere. The clinic operates from 8:00 AM to 5:00 PM, Monday through Friday, ensuring a structured yet flexible work environment where therapists can excel while maintaining a healthy work-life balance.

Key Responsibilities

• Conduct comprehensive evaluations of patients to assess functional limitations and disabilities, developing tailored treatment plans based on individual needs.
• Implement evidence-based treatment protocols that are aligned with patient evaluations and ongoing assessments while ensuring adherence to state practice acts and regulations.
• Collaborate with interdisciplinary healthcare teams to discuss patient progress and make timely adjustments to treatment plans based on patient responses and changing conditions.
• Document all patient care activities meticulously, including initial evaluations, daily progress notes, and discharge summaries, to provide a complete picture of patient care and outcomes.
• Participate in discharge planning and follow-up care to ensure continuity in rehabilitation services and identify any further healthcare needs for patients.
• Contribute to team efforts aimed at removing barriers to access and enhancing patient care delivery within the clinic.
• Engage actively in facility meetings, training sessions, and continuing education opportunities to stay informed of the latest physical therapy practices and innovations.

Patient Population / Client Focus

In this role, you will work primarily with patients recovering from orthopedic surgeries, injuries, and various physical conditions that affect their mobility and quality of life. Your expertise will be vital in guiding patients through rehabilitation processes, motivating them toward active engagement in their recovery. The focus on outpatient care allows you to cultivate lasting relationships with patients, guiding them at every step of their healing journey.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited program.
• Active state licensure as a Physical Therapist in Florida.
• At least two (2) years of clinical experience in an outpatient orthopedic setting.
• Current CPR certification to ensure high standards of patient safety and emergency response.

Preferred Experience

While two years of outpatient orthopedic experience is required, additional qualifications such as prior experience in a rehabilitation center, certifications in specialized treatment techniques, or proficiency in electronic medical records will be advantageous in our fast-paced clinical setting.
