OnTrack Physical Therapy & Wellness is seeking a dedicated and skilled Orthopedic Physical Therapist (PT) to join our team at our rehabilitation facility located in the scenic Punta Gorda, Florida. Work Environment

The rehabilitation facility in Punta Gorda is designed to provide an inviting and supportive environment for both patients and staff. Our onsite team comprises physical therapists, rehabilitation technicians, and administrative staff who work cohesively to enhance the patient experience. The typical work schedule for this position is Monday through Friday from 8 AM to 5 PM, allowing for a balanced work-life schedule.

Key Responsibilities

• Consult with other healthcare professionals to modify patient care plans as needed and document changes accurately.
• Implement evidence-based treatment protocols tailored to each patient's unique evaluation findings and healthcare needs.
• Conduct thorough evaluations, including reviewing patient histories, performance assessments, and formulating clear recommendations based on the results.
• Oversee discharge planning and follow-up care, ensuring accurate documentation of patient encounters, goals, and progress throughout the treatment process.
• Maintain compliance with all corporate and clinical training requirements and adhere to state and federal regulations.
• Evaluate, treat, and direct therapy interventions for patients requiring physical therapy, clearly articulating the medical necessity in documentation.
• Promote open communication with co-workers, physicians, patients, and third-party payors to foster an effective treatment team environment.
• Participate in facility meetings and professional development as directed by your manager.

Patient Population / Client Focus

In this role, you will primarily serve a diverse range of orthopedic patients, including those recovering from surgeries, sports injuries, and other musculoskeletal conditions. Your primary aim will be to improve their mobility, reduce pain, and enhance their overall quality of life through tailored physical therapy interventions.

Qualifications

• Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• Current state licensure as a Physical Therapist in Florida.
• American Heart Association BLS for Healthcare Providers CPR certification is a prerequisite.

Preferred Experience

A minimum of two years of clinical experience in a physical therapy setting, particularly focusing on orthopedic rehabilitation, is preferred but not mandatory. Candidates with a proven track record of utilizing holistic and patient-centered approaches in therapy will stand out.
