Work Environment

This position is primarily onsite at our patients' residences within Somerset and surrounding areas. You will have the autonomy to manage your schedule while ensuring that each patient receives timely and effective treatment. Key Responsibilities

• Conduct comprehensive in-home assessments to evaluate patients' physical capabilities and limitations, and develop personalized care plans based on their specific conditions and rehabilitation goals.
• Deliver a range of therapeutic interventions including therapeutic exercises, manual therapy, gait training, and balance retraining aimed at restoring patients' function and mobility.
• Provide education and training for patients and their caregivers regarding treatment plans and home exercise regimens to ensure continuity of care.
• Collaborate with a multidisciplinary team including physicians, nurses, and other healthcare professionals to optimize patient outcomes and ensure a holistic approach to rehabilitation.
• Maintain accurate and timely documentation of patient progress in compliance with Medicare standards and agency protocols, ensuring seamless communication among care team members.
• Travel to multiple patient homes within your assigned region, promoting flexibility and adaptability in your daily work approach.

Patient Population / Client Focus

As a Physical Therapist at Clinica Recovery, you will work with a diverse patient population ranging from post-surgical recovery individuals to those managing chronic conditions such as arthritis or neurological disorders. Your expertise will play a vital role in enabling these patients to regain independence and improve their capacity to perform daily activities.

Qualifications

• Must hold a valid license to practice Physical Therapy in the state of New Jersey.
• A Bachelor's degree in Physical Therapy from an accredited program is required; a Doctor of Physical Therapy (DPT) degree is preferred.
• Minimum of two years of experience as a Physical Therapist, preferably in a home health setting, is essential.
• Possession of a valid driver's license and reliable transportation is required; the role necessitates consistent travel to patient homes.
• Exceptional communication, time management, and interpersonal skills are necessary to effectively engage with patients and caregivers.
• Must demonstrate the ability to work independently and efficiently manage a caseload in a homecare context.

Preferred Experience

• Experience with electronic medical records (EMR) systems is preferred, enhancing documentation and communication capabilities.
• Prior experience in a home health or community-based rehabilitation setting is considered an advantage.
• Familiarity with Medicare regulations and home health policy can contribute positively to your application.

Compensation and Benefits

The compensation for this role ranges between $110,000 and $125,000 annually, commensurate with experience, plus additional benefits. Our comprehensive benefits package includes:
