Work Environment

This role is primarily onsite in a home care setting, allowing you to work directly within the community you serve. Key Responsibilities

• Conduct thorough evaluations through the review of medical history and implement specific assessments to determine a comprehensive treatment plan tailored to each patient's needs.
• Communicate evaluation findings to patients and their families, actively involving them in goal-setting to ensure alignment with their recovery aspirations.
• Implement evidence-based treatment protocols based on patient evaluations, adapting approaches as needed to meet the changing dynamics of patient care.
• Collaborate with multidisciplinary healthcare teams to provide holistic care, documenting any alterations in patients' conditions that may require changes to their plans of care.

Patient Population / Client Focus

Qualifications

• A Bachelor's Degree in Physical Therapy is required; Doctorate in Physical Therapy (DPT) is preferred.
• A valid Physical Therapist license in the State of New Jersey is mandatory.
• Current certification in Basic Life Support (BLS) from the American Heart Association is required.

Preferred Experience
