Key Responsibilities\n

• Psychosocial Assessment: Conduct thorough assessments in a timely manner, utilizing your knowledge of human behavior and psychosocial factors that affect treatment outcomes. This involves identifying ethical and risk issues, such as potential abuse or neglect.

• Clinical Intervention: Provide supportive counseling to patients and families, facilitate family conferences, and assist with difficult decisions such as end-of-life care options. Refer families to community resources to ensure a comprehensive support network.

• Caseload Management: Skillfully triage and manage a diverse range of clinical cases. Effectively balance high-risk admissions and ongoing patient care while collaborating with case managers for seamless discharge planning.

• Documentation: Maintain accurate and timely documentation according to hospital policies, including progress notes and interdisciplinary plans of care to support coordinated patient care.

• Quality Improvement: Participate in activities aimed at enhancing patient care quality, including monitoring and analyzing outcomes to drive evidence-based improvements.

• In-Service Training: Conduct educational sessions for staff on psychosocial issues affecting patient care, contributing to a supportive environment within the hospital.

Qualifications\n

• Master's degree in Social Work from an accredited graduate program.

Compensation and Benefits\n

This position offers a competitive annual salary ranging from $120,000 to $130,000, commensurate with experience and qualifications.
