This position offers a flexible remote work setting, making it easier for you to balance both your professional responsibilities and personal life. While you will primarily engage in remote work, occasional meetings and team collaborations may take place at our community health clinic located in Fremont, California. Key Responsibilities\n

As a Licensed Independent Social Worker (LISW), your key responsibilities will include:

• Conducting thorough evaluations and assessments to identify the developmental needs of children and families.

• Actively participating in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring they are tailored to each family's unique situation and needs.
\n
• Monitoring the progress and outcomes of enrolled families, providing necessary adjustments to service plans as required.
\n
• Building rapport with children, families, and multidisciplinary team members to foster a supportive and engaging learning environment.
\n
• Utilizing evidence-based practices to recommend strategies for families, promoting developmental milestones for children.
\n
• Coaching families on how to integrate developmental strategies into everyday routines, following Natural Learning Environment Practices.

• Ensuring compliance with all relevant Federal and State regulations to safeguard the health and safety of the individuals we serve.

Patient Population / Client Focus\n

The focus of your work will be primarily with children aged birth to three years who may have disabilities or developmental delays. You will support families through evaluations that help identify their child's needs and offer early intervention services that aim to enhance communication, social, emotional, cognitive, and physical skills. Your role will be significant in creating a supportive network that encourages every child to thrive in a community health setting.

Qualifications\n

The ideal candidate will possess the following qualifications:

• A Master's Degree or Doctorate in Social Work from an accredited institution.

• Current licensure as a Licensed Independent Social Worker (LISW) issued by the Department of Public Health.

• A valid driver’s license to facilitate travel for evaluations and client meetings.

• Strong organizational and time management skills to effectively handle case loads.

• Excellent communication skills to engage effectively with clients and collaborate with team members.

Preferred Experience\n

Compensation and Benefits\n
