Job Overview Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing job in Morton, Pennsylvania and surrounding areas. The role involves leading and collaborating to provide management and delivery of patient care plans for home health patients. This position supports patients in Havertown, PA and surrounding areas and is eligible for a $10,000 first‑year success bonus to reward the accomplishment of key achievements during the first year. Responsibilities Lead and collaborate with the care team to manage and deliver patient care plans. Support patients in Havertown, PA and surrounding areas. Deliver quality patient care in the home health setting, ensuring clinical competence. Consistently meet critical deadlines for charting and documentation. Self‑manage time and schedule, driving up to 20% of the day to patient locations. Qualifications RN license in the state of employment. Graduate from an approved school of practical nursing. At least one year of RN experience with clinical competence to deliver quality patient care. Current driver’s license and ability to drive to/from patient locations.
