Key Responsibilities\n

• Conduct thorough assessments of patients’ psychosocial needs, delivering tailored treatment plans that address both short-term counseling and crisis intervention.

• Assist patients in navigating administrative challenges by providing support in completing necessary documentation for entitlements and accessing community resources.

• Coordinate and facilitate access to prescription assistance programs offered by pharmaceutical companies and other community organizations, empowering patients to manage their healthcare needs effectively.

• Advocate fiercely for patients when collaborating with community agencies, ensuring they receive the comprehensive support they deserve.

• Create and maintain meticulous patient records while preparing reports and documentation that adhere to best practices in mental health care.

Qualifications\n

• A minimum of two (2) years of professional experience in a mental health counseling role is required.
