You will primarily deliver care to clients in their homes, interacting in environments that range from comfortable and familiar to those potentially requiring modification for rehabilitation. Direct the administration of physical therapy treatments focused on preventing disability, relieving pain, and restoring muscle function in line with established care plans.

Evaluate clients' condition through diagnostic and prognostic testing to inform appropriate physical therapy practices.

Provide personalized therapy services in the client's home, adhering strictly to physician prescriptions.

Administer therapeutic exercises, including manual therapies and patient mobility training, alongside non-manual activities aimed at enhancing functional independence.

Assess the home environment to recommend adaptations that support rehabilitation and independence.

Oversee the work of physical therapy assistants, ensuring compliance with care plans and providing mentorship.

Contribute to care plan development and regularly review and update plans based on clinical findings and client condition changes.

Fare vigilant in observing client progress and any conditions, communicating changes promptly to physicians and case managers.

Engage in multi-disciplinary team meetings, collaborating with various healthcare professionals to evaluate and plan comprehensive treatment modalities.

Offer guidance and education to clients and their families regarding their therapy programs and the usage of any necessary physical therapy devices.

Uphold administrative responsibilities, ensuring timely submission of clinical documentation and adherence to agency policies.
