Key Responsibilities\n

• Conduct comprehensive assessments and develop individualized treatment plans tailored to meet the unique needs of each client.

• Maintain open and effective communication between supervisory staff, direct care team members, family members, guardians, and other relevant stakeholders regarding client progress.

• Provide a detailed schedule of appointments, including home visits, to supervisors to facilitate oversight and effective tracking of client care.

• Coordinate in-home visits to perform essential observations and collect data analyzed for service delivery and improvement.
\n
• Ensure the program plan is implemented effectively, adjusting strategies based on client feedback and progress.

• Engage actively in Interdisciplinary Team Meetings, contributing insights from the counseling perspective to holistic client care.
\n
• Document and complete monthly and quarterly progress notes that reflect the evolution of each client’s treatment journey.
\n
• Organize and conduct training sessions to ensure all team members are competent in the implementation of program plans and documentation practices.
\n
• Facilitate ongoing follow-up interviews and home visits as necessary to ensure continuity of care and support for clients.
\n
• Maintain thorough and accurate records that ensure accessibility to vital information for authorized personnel.
\n
• Serve as an ambassador for the organization within the broader mental health community, fostering relationships that support the developmental goals of our clients.

• Assist in developing training materials and improving program procedures to enhance the quality of care for clients.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• An active and unrestricted licensure as a Clinical Professional Counselor (LCPC) within the state of Arizona is mandatory.
