Key Responsibilities

• Conduct comprehensive bio-psychosocial evaluations to identify patients' needs and barriers to treatment, ensuring each plan tailored to assist in overcoming personal challenges.
• Develop and implement individualized treatment plans utilizing both individual and group therapy methods, adapting approaches based on each patient's circumstances.
• Collaborate with a multidisciplinary team of healthcare providers to ensure cohesive care delivery, providing crucial insights into patients' psychosocial functioning to improve treatment adherence.
• Document social work activities and patient interactions meticulously in medical records, ensuring compliance with departmental policies, applicable laws, and ethical standards.
• Educate patients and their families on health recommendations, fostering understanding and acceptance towards restoration of function and well-being.
• Advocate for patients and their families within community resources, promoting self-determination while strengthening social networks.
• Maintain current knowledge of community resources and continuously enhance clinical expertise through ongoing education and training opportunities.
• Contribute to departmental goals through accurate reporting of case-related statistics in alignment with quality improvement measures.

Patient Population / Client Focus

You will primarily work with a diverse demographic that includes individuals experiencing a range of mental health challenges, including anxiety, depression, trauma, and interpersonal conflicts. Qualifications

• A Master’s Degree in Social Work from an accredited institution is required.
• Must possess a current Licensed Clinical Social Worker (LCSW) license from the State Board of Social Work Licensure.

Preferred Experience

Compensation and Benefits

The role offers a competitive salary range between USD 85,000 and $100,000 yearly, commensurate with experience and qualifications.
