Work Environment\n

This position is entirely remote, offering flexibility that caters to work-life balance. You will collaborate with a multidisciplinary team of healthcare professionals, including medical doctors, nurses, and other therapists. Despite the remote nature of the job, you will engage in regular virtual team meetings to discuss cases, share insights, and develop coordinated care plans. The working hours are structured from 9 AM to 6 PM, Monday through Friday, allowing you to dedicate your evenings and weekends to personal time.

Key Responsibilities\n

• Conduct comprehensive assessments of clients' mental health needs to develop personalized treatment plans.

• Engage in the evaluation of patients through intake procedures, ensuring a timely and respectful approach to care.

• Manage a caseload of clients experiencing a range of mental health disorders, providing case management and behavioral health services.

• Perform crisis interventions and therapeutic counseling for clients facing acute psychological challenges.

• Expertly navigate and maintain adherence to mental health policies and procedures, ensuring high-standard care delivery.

• Facilitate referrals for patients with severe mental health issues that require inpatient or specialty care services.

• Screen for substance abuse disorders, including prescription drug, illicit drug, and alcohol abuse, guiding patients to appropriate resources.

• Participate actively as a member of the primary care team, collaborating effectively with healthcare colleagues to optimize patient outcomes.

The LCSW will primarily work with hospice patients and their families, addressing the emotional and psychological complexities associated with end-of-life care.
