Key Responsibilities

• Conduct thorough evaluations of patients, including assessing medical histories, performing targeted physical assessments, and making informed recommendations for treatment.
• Clearly communicate evaluation findings and treatment plans to patients and their families, actively involving them in goal-setting to enhance motivation and adherence.
• Develop and implement personalized treatment protocols based on each patient's unique needs, ensuring adaptability to their health status and concerns.
• Collaborate with interdisciplinary healthcare teams, recognizing critical changes in patients' conditions and documenting necessary adjustments to care plans, when required.
• Manage discharge planning processes effectively, ensuring that follow-up care is seamless and appropriately documented, showcasing a comprehensive care narrative that includes evaluations and progress notes.
• Maintain efficient time management and productivity, adhering to scheduling commitments and attending meetings punctually, as well as engaging in community marketing initiatives.
• Contribute proactively to the achievement of departmental goals and participate in compliance with regulatory standards.
• Engage in continuous professional development, taking the initiative in pursuing further education and serving as a clinical instructor to students when applicable.
• Foster an exceptional patient experience by being adaptable and responsive to their needs and facilitating their access to care.
• Participate in team-oriented initiatives to improve access to rehabilitation services and understand relevant performance data to enhance care delivery.

Patient Population / Client Focus

As a Physical Therapist at Bountiful Life, you will work predominantly with outpatient clients recovering from a variety of conditions, including sports injuries, orthopedic surgeries, neurological disorders, and chronic pain. Our patient population ranges from pediatric to geriatric, emphasizing personalized care plans tailored to meet individual recovery goals and enhance their daily living activities.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution is required.
• Must hold a valid DPT license in the state of Virginia.
• American Heart Association's Basic Life Support (BLS) for Healthcare Providers certification is mandatory.
• All candidates should have excellent interpersonal and communication skills, with a commitment to teamwork and patient-centered care.

Preferred Experience

• While a minimum of 2 years of experience in physical therapy is preferred, we welcome dedicated new graduates with a passion for learning.
• Experience in outpatient therapy settings and familiarity with electronic health records (EHR) are considered advantageous.
