Work Environment

This role is primarily onsite, focusing on home care for our patients in Naples and the surrounding areas. You will work closely with other healthcare professionals, including nurses and physicians, in a warm, community-oriented setting that values teamwork. Key Responsibilities

• Conduct comprehensive evaluations, gathering pertinent medical history and performing specific assessments to establish a baseline and identify treatment goals.
• Communicate evaluation findings and treatment plans to patients and their families, ensuring active participation in the goal-setting process.
• Implement individualized treatment protocols based on assessment results, patient presentations, and best practices in rehabilitation.
• Regularly consult with multidisciplinary healthcare teams regarding patient care, documenting any changes in conditions that may require modifications to treatment plans.
• Oversee discharge planning and follow-up care to help patients maintain progress post-therapy.
• Document patient interactions accurately and promptly, ensuring compliance with evolving healthcare regulations, and contributing to patient education and understanding.
• Manage your schedule effectively to ensure timely and productive patient interactions while meeting departmental productivity expectations.
• Participate in hospital and community marketing initiatives to raise awareness of our services.
• Engage in ongoing professional development and continuing education to enhance personal competencies and organizational capabilities.
• Support team efforts in identifying and removing barriers to access to care, actively participating in data analysis and quality improvement initiatives.

Patient Population / Client Focus

The patients you will serve predominantly include individuals recovering from orthopedic surgeries, sports injuries, and chronic pain conditions. Many of our patients are older adults requiring specialized rehabilitation interventions to regain independence. You will utilize a patient-centered approach, ensuring that care is tailored to the unique circumstances and personal goals of each individual.

Qualifications

• Bachelor's degree in Physical Therapy or higher, with a Doctorate in Physical Therapy preferred.
• State licensure as a Physical Therapist as required by Florida law.
• Current certification in Basic Life Support (BLS) through the American Heart Association.
• Strong interpersonal skills to foster relationships with patients and their families.
