Work Environment

This position is an onsite role providing home health services directly to patients in their residences. You will work closely with patients in a familiar environment, fostering a supportive atmosphere while promoting optimal therapeutic outcomes. Key Responsibilities

• Conduct comprehensive evaluations of patients experiencing functional limitations and develop individualized treatment plans tailored to their specific needs.
• Implement evidence-based interventions while assessing patient progress through regular re-evaluations and documentation.
• Collaborate with physicians and interdisciplinary teams to ensure holistic and effective patient care, including drafting clinical notes and treatment recommendations.
• Educate patients and their families about their conditions, rehabilitation techniques, and home exercise programs to enhance recovery.
• Maintain accurate records of all treatments and outcomes, ensuring compliance with legal, ethical, and professional standards.
• Engage in quality improvement initiatives, community outreach programs, and supervise support staff effectively to optimize department resources.
• Participate actively in interdisciplinary team meetings, contributing insights based on patient assessments and treatment outcomes.
• Stay current with industry best practices through ongoing professional development and participation in in-service training sessions.

Patient Population / Client Focus

The primary focus of this role will be on managing patients with orthopedic conditions, including post-surgical rehabilitation, musculoskeletal injuries, and chronic pain management. Qualifications

• A Bachelor's degree in Physical Therapy or a Doctorate in Physical Therapy is required.
• Active state licensure as a Physical Therapist in Florida.

• Experience working in home health and orthopedic rehabilitation is highly desirable.

Preferred Experience

Compensation and Benefits

We offer a competitive salary ranging from USD 100,000 to 120,000 per year, commensurate with experience and qualifications.
