Work Environment

This full-time position requires an onsite presence, primarily focused on home care, interacting directly with patients in their residences. Our therapists work within a supportive team environment, fostering collaboration to achieve the best possible patient outcomes. The role offers a structured Monday through Friday schedule with a 40-hour work commitment, allowing for work-life balance while meeting the needs of our patients.

Key Responsibilities

• Evaluate, plan, and implement individual treatment programs for patients referred by physicians, ensuring tailored interventions that meet specific recovery goals.
• Deliver a wide range of physical therapy services, engaging with patients who have orthopedic, pediatric, neurologic, and sports medicine needs.
• Utilize state-of-the-art procedures and therapeutic techniques to effectively change patients' conditions in line with physicians' care plans.
• Educate and train patients and their caregivers, ensuring understanding of treatment protocols and empowering them in their recovery journey.
• Demonstrate outstanding customer service, contributing positively to team dynamics and organizational effectiveness.
• Work collaboratively with interdisciplinary teams to maximize patient outcomes using research-driven methodologies.
• Adhere to and strive to meet minimum utilization goals and other business objectives, ensuring the success of our service delivery.

Patient Population / Client Focus

As a Home Health Physical Therapist, you will engage with a diverse patient population, focusing on individuals recovering from surgery, managing chronic conditions, or seeking rehabilitation after injuries. Your role is pivotal in achieving their recovery milestones through personalized care in the comfort of their homes, enhancing their functional independence.

Qualifications

• Current Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A valid State License in Physical Therapy to practice in Virginia.
• Minimum of two years of experience in a clinical setting, preferably in home health or outpatient therapy.
• Basic Life Support Certification from the American Heart Association.
• Proficient written and verbal communication skills for effective interaction with patients and healthcare teams.
• Strong computer skills to maintain accurate patient records and navigate electronic health record systems.
• Ability to build strong, professional relationships with patients, showcasing empathy and understanding to motivate patients in achieving their functional goals.
• Desire to engage positively with clients and contribute to community health initiatives.

Preferred Experience

• Experience in developing and executing evidence-based treatment plans tailored to individual patient needs.
• Background in specialized therapy modalities, such as dry needling, aquatic therapy, or manual therapy.
• Familiarity with software and tools for telehealth and virtual therapy sessions, enhancing patient engagement.

Compensation and Benefits
