Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and challenges.

• Develop, implement, and monitor care plans using evidence-based social work interventions tailored to the unique needs of each patient.

• Collaborate effectively with the healthcare team, including physicians and nursing staff, to integrate psychosocial care with medical treatment.

• Facilitate patient discharge planning by coordinating with consultants, insurers, and service providers to ensure a smooth transition after hospitalization.

• Offer educational sessions and counseling to patients and families, focusing on adaptation to illness, coping strategies, and post-hospitalization support.

• Utilize a range of therapeutic techniques, such as crisis intervention, individual therapy, and family therapy, adjusting the approach based on patient needs.

• Act as a resource for other healthcare staff, providing guidance and education on psychosocial issues.
\n
• Engage with patients experiencing oncology-related challenges, offering clinical evaluations and short-term counseling for depression and anxiety.

• Participate in on-call rotations to respond to urgent after-hours situations as needed.
\n
• In instances requiring immediate attention, conduct involuntary examinations under the state’s Baker Act, ensuring patients receive appropriate care.

• Present your expertise in psychosocial oncology at conferences and community events, advocating for the psychosocial needs of cancer survivors.

• Contribute to community services through education, outreach, and collaboration with local organizations supporting cancer patients.

Patient Population / Client Focus\n

The primary patient population includes individuals facing mental health challenges, particularly those related to hospitalization, chronic or acute illness, and cancer diagnoses. You will provide vital psychosocial support tailored to enhancing their emotional well-being and facilitating their recovery journey.

Qualifications\n

The ideal candidate will possess a Master’s degree in Social Work from an accredited program, along with a valid Licensed Independent Clinical Social Worker (LICSW) credential. Preferred Experience\n
