Key Responsibilities

• Design and implement individualized physical therapy treatment plans tailored to the specific functional abilities and goals of each patient.
• Conduct thorough evaluations to assess patients' physical condition, utilizing diagnostic tests related to muscle, nerve, joint, and functional abilities.
• Direct and supervise the activities of physical therapy assistants to ensure adherence to the care plans established for each client.
• Provide therapeutic interventions including manual therapy techniques, therapeutic exercises, and modalities such as ultrasound and electrical stimulation.
• Monitor patient progress and adjust treatment plans as necessary to align with patient goals and therapeutic outcomes.
• Collaborate as part of an interdisciplinary team during case conferences to evaluate and adapt collective treatment plans.
• Engage with patients and their families to educate them about physical therapy protocols, home exercises, and the use of assistive devices.
• Maintain accurate clinical records including evaluations, progress notes, and discharge summaries, ensuring compliance with documentation standards.
• Participate in quality improvement initiatives to enhance the efficacy of therapy services provided.
• Stay updated with the latest advancements in physical therapy practices and incorporate evidence-based methodologies in treatment.

Patient Population / Client Focus

You will be serving a diverse patient population, including individuals recovering from surgery, managing chronic conditions, and addressing injuries. Our clients range from young adults to the elderly, with unique rehabilitation needs. Your role is vital in assisting each patient in achieving optimal functionality and autonomy in their daily activities.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited educational institution.
• Must possess an active state license to practice as a Doctor of Physical Therapy (DPT).
• Experience in physical therapy, particularly within an acute care or home care setting, is essential.
• Creative thinker with a proactive approach to problem-solving and decision-making.
• Strong communication skills to effectively engage with patients, families, and team members.

Preferred Experience

• Minimum of one year of relevant experience in a physical therapy role.
