Key Responsibilities\n

• Conduct thorough psychosocial assessments to understand the complexities of clients’ mental health and emotional needs, examining factors such as their medical history, present circumstances, and support systems.

• Utilize a variety of evidence-based therapeutic interventions, including cognitive-behavioral therapy and solution-focused therapy, to provide individual, family, and group counseling.
\n
• Develop personalized treatment plans and facilitate their implementation, ensuring that all interventions are aligned with clients' goals and preferences.
\n
• Engage in crisis intervention when necessary, providing immediate support and safety planning for clients experiencing acute emotional distress.
\n
• Maintain detailed, accurate, and timely documentation of client interactions, adhering to ethical and legal standards set forth by state regulations.

• Facilitate psychoeducation classes and support groups, empowering clients and their families with tools and knowledge to navigate their emotional journeys.

• Collaborate closely with healthcare colleagues—including nurses, physicians, and psychologists—to ensure seamless, integrated care for all clients.

Patient Population / Client Focus\n

Your work will primarily involve clients who are receiving end-of-life care or who are experiencing severe illness. You will assist these individuals and their families in coping not only with the physical aspects of illness but also with the emotional challenges they may face, guiding them toward acceptance, healing, and support during their final life stages.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of direct experience in a mental health or outpatient setting, with a focus on the geriatric population preferred.

• Proficient knowledge of evidence-based therapeutic techniques and a demonstrated ability to implement them in clinical practice.

• Exceptional interpersonal and communication skills, with a keen ability to assess and relate to clients from diverse backgrounds.

Compensation and Benefits\n
