Key Responsibilities\n

As a Licensed Clinical Professional Counselor at Crystal Lake Families & Kids Services, you will:

• Work collaboratively with primary care providers to identify, manage, and treat psychosocial and mental health issues affecting patients.

• Conduct thorough patient intakes, focusing on diagnostic evaluations, and develop personalized treatment goals in partnership with the patient.

• Provide evidence-based therapeutic interventions through brief sessions tailored to the unique needs of each individual.

• Advise primary care providers on appropriate care settings for different patient needs, ensuring timely referrals when necessary.

• Regularly follow up with patients to track their progress and adjust treatment plans as required, providing guidance for sustained recovery.

• Create relapse prevention plans and strategies that empower patients towards achieving stable mental health and emotional resilience.

• Document patient progress meticulously within treatment charts and maintain open communication with primary care teams regarding patients' needs and treatment outcomes.

• Assist in identifying at-risk patients and proactively engage in the development of preventive strategies.

• Educate patients on self-management techniques and set goals for healthy lifestyle changes.

• Participate in team meetings and contribute insights regarding behavioral health services and program development.

• Maintain strict adherence to the Code of Ethical Conduct for Clinical Professionals.

Qualifications\n

Compensation and Benefits\n

Crystal Lake Families & Kids Services offers a competitive salary range of USD 87,000 – 105,000 annually commensurate with experience. In addition to a comprehensive benefits package that includes health insurance, paid time off, and retirement plans, we also provide a SIGN-ON BONUS for qualified candidates. Counselors will be encouraged to pursue continuous professional development and obtain additional certifications relevant to their practice.
