Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for patients diagnosed with cancer, evaluating emotional, social, and environmental factors impacting their health.

• Provide ongoing supportive counseling to patients and their families, from the moment of diagnosis through various treatment phases, to alleviate stress and enhance coping mechanisms.

• Utilize evidence-based therapeutic interventions to address acute patient needs during times of crisis, facilitating emotional stabilization and resilience.

• Develop and implement personalized care plans that integrate social support and community resources, coordinating with interdisciplinary teams to improve patient outcomes.

• Monitor and support patients during hospital stays, ensuring continuity of care and providing emotional reassurance to both patients and their families.

• Maintain accurate documentation and compliance with our organization’s Code of Conduct, addressing any compliance questions or concerns promptly.

Patient Population / Client Focus\n

This role primarily serves cancer patients and their loved ones within the hospice setting. Qualifications\n

Compensation and Benefits\n
