Key Responsibilities\n

As a Licensed Professional Counselor at Mind Crafts Counseling Center, you will:

• Conduct comprehensive and ongoing assessments to understand and address the unique needs of each patient.

• Facilitate effective communication among supervisors, direct care staff, family members, and other stakeholders to keep all parties informed about individual progress.

• Devise and share a tentative schedule of appointments, home visits, and necessary observations for effective monitoring of patient advancement.

• Organize and coordinate home visits that may include in-home observations, chart reviews, and staff interviews to gather essential data for patient care.

• Ensure the effective implementation of individualized program plans and make necessary adjustments based on ongoing evaluations.
\n
• Participate actively in interdisciplinary team meetings, promoting collaboration and sharing insights that can enhance patient care.
\n
• Maintain detailed records and complete monthly and quarterly progress notes to provide clear documentation of client progress and treatment outcomes.

• Provide training and support for staff members to ensure competency in the implementation of program plans and relevant documentation processes.

• Conduct follow-up interviews, home visits, and observations to provide continued support tailored to individual needs.

• Acts as a liaison with the broader mental health community to advocate for clients' development goals and resource needs.

• Assist in creating comprehensive training materials that enhance program effectiveness and staff competencies.

Qualifications\n
