This position is fully remote, allowing for flexible work schedules that accommodate both our patients and staff. While most services will be delivered virtually, there may be times when in-person visits to the hospice center are necessary to provide comprehensive care. Key Responsibilities

• Utilize your education and expertise to conduct thorough psychosocial assessments and diagnoses of patients, forming strong foundational relationships built on trust and empathy.
• Collaborate with patients to create personalized treatment plans that address their unique psychological and emotional needs, enhancing their quality of life during terminal illness.
• Employ evidence-based interventions such as solution-focused therapy, motivational interviewing, and behavioral activation to help patients manage their emotional challenges effectively.
• Work closely with a multidisciplinary team, including psychologists and primary care providers, to establish an integrated care approach that promotes overall well-being.
• Monitor patient progress through systematic tracking of treatment responses, ensuring consistent follow-up via virtual or in-person sessions to evaluate effectiveness and adjust treatment as necessary.
• Assist with psychotropic medication management by supporting adherence, tracking side effects, and facilitating communication between patients and medical providers regarding treatment efficacy.

Patient Population / Client Focus

The primary clientele includes patients in hospice care, who are often navigating the complexities of terminal illnesses. Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• Current and valid LCSW licensure to practice independently in the state of Georgia.

Compensation and Benefits
