Work Environment

The work setting is primarily onsite in the home care environment, where you will visit patients in their homes to provide therapy services. This approach allows for a personalized treatment experience that aligns with patients' everyday routines and lifestyles. You will work Monday to Friday from 9 am to 5 pm, ensuring a work-life balance while engaging directly with patients in a comforting setting.

Key Responsibilities

• Conduct comprehensive assessments to identify patients' goals and develop personalized treatment plans aimed at restoring mobility and function.
• Manage a caseload efficiently, working with one patient per hour to ensure dedicated, individualized attention.
• Utilize evidence-based practices to implement interventions that improve patients' physical capabilities while effectively reducing reliance on medications or surgical procedures.
• Design and deliver tailored fitness and wellness programs that align with patients' specific needs, focusing on enhancing strength, flexibility, balance, and coordination.
• Monitor and document treatment outcomes, adjusting care plans as necessary to optimize progress and maintain clear communication with patients and their families.
• Collaborate with multidisciplinary healthcare professionals to ensure a holistic approach to patient care.
• Maintain meticulous documentation of patient interactions and treatment services using EMR software, specifically WebPT.
• Encourage and motivate patients throughout their recovery journey, fostering a supportive atmosphere that enhances their commitment to rehabilitation.

Patient Population / Client Focus

In this role, you will primarily work with patients recovering from orthopedic conditions, including post-surgical rehabilitation, sports injuries, and age-related mobility issues. The patient population includes diverse demographics, necessitating a compassionate, adaptable approach that is sensitive to the nuances of each patient's background and lifestyle.

Qualifications

• A Bachelor's Degree in Physical Therapy is required. A Master's or Doctoral Degree in Physical Therapy is preferred.
• Must possess a valid state license as a Physical Therapist with a clean professional record, free from any disciplinary actions.
• Proficiency in manual therapy techniques and the development of individualized exercise protocols is essential.
• Experience utilizing EMR software, particularly WebPT, to track patient progress and document care.

Preferred Experience

• Prior experience in orthopedic outpatient settings, demonstrating a solid understanding of rehabilitation protocols and patient care strategies.
• Demonstrated ability to engage patients with diverse backgrounds through effective communication and motivational techniques.
• Experience in developing comprehensive treatment plans based on evidence-based practices and ongoing evaluation of patient progress.

Compensation and Benefits
