Work Environment

This position primarily takes place in the homes of our patients throughout the Middletown area. Your role will involve traveling to various locations, where you will implement tailored therapy plans in comfortable, familiar settings. Key Responsibilities

• Conduct comprehensive assessments of patients, evaluating their physical abilities and limitations to formulate effective treatment plans.

• Provide ongoing evaluation of patient progress, making necessary adjustments to treatment plans as required.
