Work Environment

This position is onsite and will require traveling to patients' homes within Camden and surrounding areas. In this role, you will work closely with patients in their home environments, addressing their individual needs in a familiar setting. Key Responsibilities

• Conduct thorough examinations and evaluations of patients to identify functional limitations and design tailored treatment plans.
• Deliver patient care with adherence to customer service standards, ensuring respectful and effective communication with individuals of all ages and backgrounds.
• Perform treatment interventions as outlined in patient plans, regularly monitoring progress and making necessary adjustments to maximize outcomes.
• Document all patient interactions and service provisions accurately in both written and electronic formats, complying with relevant laws, rules, and regulatory standards.
• Engage in interdisciplinary collaboration, effectively liaising with other healthcare providers, referral sources, and stakeholders to enhance the continuum of care.
• Lead or participate in community outreach initiatives and internal marketing efforts that promote our services and educate the public about physical therapy benefits.
• Provide education to patients and their families about treatment processes and home care strategies, helping them integrate therapeutic exercises into their daily routines.
• Participate in leadership functions, mentoring less experienced staff, and supervising therapy aides and students in clinical settings.
• Regularly attend and contribute to team meetings, sharing insights and participating in quality improvement activities.
• Maintain licensure and certification in Physical Therapy, committing to lifelong learning through ongoing professional development.

Patient Population / Client Focus

As a Physical Therapist at Serene Harmony Therapy Center, you will work with a diverse patient population, focusing on individuals recovering from surgeries, strokes, orthopaedic conditions, and other debilitating health issues. Qualifications
