Our team consists of licensed physical therapists who collaborate to create a patient-centered environment, ensuring each individual receives personalized attention tailored to their unique needs.

Work Environment

This position will require you to work onsite in patients' homes, allowing you to provide therapy in a comfortable and familiar setting. The full-time schedule is from Monday to Friday, 9:00 AM to 5:00 PM, allowing for structured sessions while promoting work-life balance. Our home care model means you will travel to various households primarily within the local community, where you will assess and facilitate rehabilitation programs based on each patient's specific circumstances.

Key Responsibilities

• Assess, diagnose, and develop tailored treatment plans based on each patient's needs, ensuring all therapy goals are patient-centered.
• Utilize expert knowledge in manual therapy techniques to enhance mobility and pain management for patients.
• Implement structured exercise programs, ensuring exercises are appropriate for each patient's physical capabilities and rehabilitation goals.
• Provide targeted interventions in various contexts, including education on injury prevention, activity modification, and home exercise regimens.
• Encourage and motivate patients to engage actively in their treatment plans and achieve optimal health outcomes.
• Document treatment sessions meticulously, tracking progress and modifying care plans as necessary based on ongoing evaluations.
• Collaborate with interdisciplinary healthcare teams to ensure comprehensive patient care and to streamline communication regarding treatment outcomes and adjustments.
• Stay informed on the latest research and techniques within the field of orthopedic therapy to provide evidence-based care to patients.

Patient Population / Client Focus

You will predominantly work with individuals recovering from orthopedic injuries, surgeries, and chronic pain conditions. By addressing the specific needs of each patient, you will help them regain functional independence and improve their physical capabilities. Your role will significantly impact their journey to recovery, enhancing their strength, balance, and overall well-being.

Qualifications

• A Master's or Doctorate degree in Physical Therapy (MPT or DPT) is required.
• Valid and active Physical Therapist license in the state of New Jersey.
• A minimum of three (3) years of clinical experience in physical therapy, specifically within an orthopedic outpatient setting.
• No history of board disciplinary actions or license revocation.
• Proficiency in manual therapy techniques and a strong understanding of exercise prescription and rehabilitation practices.

Preferred Experience

• Experience in home healthcare settings is an advantage, as well as familiarity with patient communication and care coordination.
• Demonstrated ability to work collaboratively with other healthcare professionals, ensuring comprehensive patient care.

Compensation and Benefits
