Work Environment

This role is designed for dedicated professionals who thrive in an onsite home care environment. You will work closely with patients in their homes, providing personalized therapy that resonates with their daily lives. The typical work schedule runs Monday through Friday from 9 AM to 5 PM, allowing for a balanced work-life experience.

Key Responsibilities

• Conduct comprehensive evaluations of patients' physical capabilities and design individualized treatment plans to bridge their unique health goals.
• Administer skilled physical therapy services, including therapeutic exercises, manual therapy techniques, and various treatment modalities, tailored specifically for geriatric patients.
• Regularly track and document patient progress through electronic medical records (EMRs) and provide timely adjustments to treatment plans when necessary to ensure optimal health outcomes.
• Engage in interdisciplinary collaboration with physicians, occupational therapists, and speech therapists to create a holistic approach to patient care.
• Educate patients and their families on therapeutic methods, home exercise programs, and strategies to prevent injuries.
• Implement effective fall prevention programs, enhancing safety measures to mitigate risks within the living environments of elderly clients.
• Commit to ongoing professional development by staying updated with advancements in physical therapy through continuing education opportunities.
• Adhere to the highest ethical standards in practice, maintaining patient confidentiality and consistently providing quality care.

Patient Population / Client Focus

You will primarily focus on our geriatric clientele, understanding and responding to their multifaceted needs. This demographic often faces unique challenges, including limited mobility, chronic pain, and the need for specialized support systems to maintain their independence. Your role is essential in facilitating their recovery and enhancing their quality of life through tailored therapeutic intervention.

Qualifications

• A graduate from an accredited college or university with at least a Bachelor's degree in Physical Therapy. Advanced degrees (Doctorate or Master's) are valued.
• An active state licensure as a Physical Therapist is required.
• Strong interpersonal and communication skills, which are crucial for effectively engaging with patients, their families, and the multidisciplinary healthcare team.
• Proficiency in using electronic medical records (EMR) and documentation systems to streamline patient care.

Preferred Experience
