Work Environment

This position is fully remote, allowing you to manage your schedule while effectively serving clients in the Galveston, Texas area. You will be associated with our Hospice Center, providing essential mental health services to clients receiving end-of-life care. Key Responsibilities

• Directly engage with clients who are experiencing major mental health issues or severe emotional disorders through individual, group, and family therapy sessions, both in office settings and the community.
• Create and implement comprehensive care plans that address the unique needs of clients, ensuring seamless access to necessary services and support systems.
• Conduct evidence-based screenings and assessments, utilizing clinical judgement to identify the most effective treatment pathways.
• Monitor and evaluate client progress, adjusting treatment plans as necessary to achieve desired outcomes.
• Report and document client interactions, treatment plans, and progress in compliance with organizational standards and ethical guidelines.
• Participate in crisis intervention when necessary, demonstrating a thorough understanding of risk management and client safety protocols.
• Collaborate with healthcare professionals across disciplines to foster integrated care approaches, ensuring comprehensive support for client needs.
• Facilitate on-call support during program hours, bringing expertise to urgent situations when required.
• Participate in committees focused on improving service delivery, quality, and community health initiatives.

Qualifications

• A Master’s Degree in Social Work (MSW) from an accredited institution is a mandatory requirement.
• A valid License as a Clinical Social Worker (LCSW) in the state of Texas is required.
• Expertise in mental health assessment, diagnosis, and treatment methodologies pertinent to the hospice care context.
• Strong written and verbal communication skills, with particular emphasis on building rapport with clients and collaborating effectively with team members.
• Demonstrated capacity for managing diverse client needs with an emphasis on equity and inclusion.
• Strong organizational skills with an ability to prioritize caseloads effectively.

Preferred Experience

• A minimum of 2 years experience in a clinical setting, preferably working with clients in end-of-life care or hospice environments.
• Experience in conducting crisis interventions and working with multidisciplinary teams.
• Familiarity with electronic health record systems for documentation and case management.

Compensation and Benefits

This position offers a competitive annual salary ranging from USD 70,000 to 85,000, commensurate with experience and qualifications.
