Work Environment\n

As a fully remote position, this role provides you with the flexibility to work from home while still engaging with a diverse patient population. You will collaborate with a multidisciplinary team comprising physicians, nurses, and support staff, ensuring a cohesive approach to patient care. Our hospice center uses advanced telehealth technologies to facilitate therapy sessions and enhance communication with patients and their caregivers.

Key Responsibilities\n

• Conduct thorough assessments of patients and their families to identify mental health needs and develop individualized care plans.

• Regularly evaluate the effectiveness of care plans and make necessary adjustments in collaboration with patients and caregivers.

• Provide crisis counseling and therapeutic interventions, conducting both individual psychotherapy sessions and group therapy sessions tailored to patient needs.
\n
• Engage with clients using evidence-based practices and methodologies to address emotional, behavioral, and social needs.
\n
• Document all patient interactions and progress toward goals in compliance with the organization's policies and mental health standards, ensuring minimal paperwork to allow more time for patient care.

• Facilitate communication among the interdisciplinary team to ensure a unified approach to patient treatment, attending regular meetings to discuss patient progress and any challenges.
\n
• Utilize therapeutic tools and software to track patient progress, outcomes, and adjustments in care plans.
\n
• Establish specific, measurable goals for each patient and continuously monitor their advancement toward those goals, adapting strategies as necessary.

• Demonstrate sensitivity to the diverse backgrounds and needs of patients and their families, providing culturally competent care that respects individual differences.

Patient Population / Client Focus\n

You will engage with a diverse patient population, including individuals experiencing serious mental health issues, substance use disorders, and other behavioral health diagnoses. Your role will be particularly focused on patients in hospice care, where you will address mental health concerns related to end-of-life issues, grief support, and emotional well-being. Your compassionate approach will help ease the psychological burdens of these patients and their families during challenging times.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution, recognized by the Council on Social Work Education.

• Active and valid Social Work License as per state regulations (LMHC, LMFT, LICSW).

• Proficiency in conducting assessments, developing treatment plans, and providing therapeutic interventions.

• Strong communication skills, both written and verbal, with the ability to engage effectively with individuals from all backgrounds.

• Ability to work independently in a remote setting, demonstrating self-motivation and time management skills.

Compensation and Benefits\n

We offer a competitive salary range of USD 80,000 to 122,000 per year, commensurate with experience and qualifications. In addition to financial compensation, benefits include:
