Work Environment\n

As a Clinical Social Worker, you will primarily engage with clients remotely but within the framework of our hospice center. You will have the unique opportunity to collaborate with a multidisciplinary team of healthcare professionals, enabling you to contribute to comprehensive care planning and holistic support for patients and their families. Your work will typically follow a Monday to Friday schedule from 8:00 AM to 4:30 PM, with occasional evening or weekend hours based on client needs.

Key Responsibilities\n

As a Licensed Clinical Social Worker, your day-to-day responsibilities will include:

• Conducting thorough assessments of clients' needs, developing and implementing individualized treatment plans, and providing therapy to address emotional and mental health challenges.

• Working collaboratively with an interdisciplinary team, including physicians, nurses, and other mental health professionals, to ensure seamless care delivery and support for clients and their families.

• Facilitating group therapy sessions and individual counseling, employing evidence-based practices to promote recovery and empowerment.

• Maintaining comprehensive documentation of client interactions, treatment plans, and progress, ensuring compliance with organizational standards and ethical guidelines.

• Engaging in crisis intervention and risk assessment when necessary, utilizing expert knowledge in managing high-stress situations and supporting clients in navigating their emotional challenges.

Patient Population / Client Focus\n

Your primary focus will be on clients experiencing significant mental health challenges, including those within hospice care settings. You will work with individuals and families facing life-limiting illnesses, providing counseling and support that is sensitive to their unique emotional, cultural, and spiritual needs. Qualifications\n

The ideal candidate for this position will possess the following qualifications:

• Current Illinois LCSW licensure is required.
