Work Environment

This role will be primarily conducted in a home care setting, allowing you to create a comfortable and familiar atmosphere conducive to rehabilitation. Your work schedule will be Monday to Friday from 9 AM to 5 PM, providing a stable work-life balance while allowing ample time to devote to each patient's complete care process.

Key Responsibilities

• Conduct thorough patient assessments to evaluate their physical capabilities and needs, ensuring you identify the best intervention strategies.
• Develop and implement tailored care plans in collaboration with physicians and multidisciplinary teams, focusing on achievable goals and patient engagement.
• Deliver in-home therapeutic interventions that utilize advanced procedures and techniques, helping patients navigate their rehabilitation effectively.
• Educate patients and their families on prescribed exercise routines, therapeutic modalities, and the proper use of assistive devices for optimal recovery.
• Regularly assess and document patient progress in accordance with professional standards, utilizing electronic medical record (EMR) software for documentation efficiency.
• Manage your caseload effectively, prioritizing daily tasks, coordinating assignments, and ensuring seamless transitions for patient care.

Patient Population / Client Focus

You will work with a diverse patient population that includes individuals recovering from orthopedic surgeries, sports injuries, and chronic musculoskeletal conditions. Your empathetic approach will be critical in fostering strong relationships, building trust, and motivating patients throughout their rehabilitation journeys.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.
• Minimum of two years of experience in an orthopedic setting, demonstrating your proficiency in delivering specialized care.
• Current state licensure as a Physical Therapist in Florida, confirming your compliance with regulatory requirements and commitment to ongoing professional development.
• Demonstrated ability to perform manual therapy techniques and develop individualized treatment plans to maximize patient recovery.
• Experience with EMR systems, particularly WebPT, to ensure accurate and timely documentation of patient information.

Preferred Experience

• Previous experience in home health care settings is highly desired, as it equips you with essential skills for working independently and assessing patient needs in their familiar environments.
